The traditional Heinrich's triangle can be modified to include the form and context of workers' activities as precursors to serious and fatal incidents. A precursor to serious and fatal incidents is defined as a high-risk situation in which management controls are absent, ineffective, or not followed, and which will result in serious injury or fatality, if allowed to continue.
